Union County Vital Records registers home births with North Carolina Vital Records so that a certified birth certificate can be issued to you.
To register your child’s certificate of birth with Union County, the birth must have occurred in Union County and the child must be less than one year old.
Home birth registrations are by appointment only.

An Affidavit of Parentage is a voluntary, legally binding statement of paternity signed by the parents of a child. Union County Vital Records processes and submits Affidavits of Parentage to North Carolina Vital Records.
To process an Affidavit of Parentage, both parents must be present and have a valid ID.

Burial Transit Permits
Burial Transit Permits are required for a funeral director or person acting as such to transport a decedent across state lines.
Note: A notification of death is required for a burial transit permit to be issued. Information provided on forms must be complete and correct.
A burial transit permit can only be issued by:
- The Local Registrar or Deputy Registrar in the County where the death occurred: Union County Human Services | Public Health - Vital Records
- An authorized sub-registrar: after business hours.
- Medical Examiner in certain instances. When a Medical Examiner completes the death certificate, a copy of that certificate serves as a burial transit permit.
The Union County Local Registrar, a Deputy Registrar or a Sub-Registrar will review, verify notification of death, sign and issue the burial transit permit electronically via secure email or in-person upon request.

Disinterment/Reinterment
An application for a Disinterment/Reinterment Permit must be completed when relocating a grave from one location to another.
- If the decedent’s remains are being moved out of North Carolina, a Burial Transit Permit is also required.
How to Access the Disinterment/Reinterment Permit Application Form:
- The legally authorized individual(s) must complete the applicable section of the Permit for Disinterment/Reinternment. This must be done in person by coming to Union County Human Services building and meeting with Public Health - Vital Records.
- The Union County Local Registrar or a Deputy Registrar is authorized to issue the Disinterment/Reinterment Permit after the required information is provided and reviewed. Please note that the signature of the Licensed Funeral Director is required on the permit and the permit will not be issued by the Local Registrar without the Licensed Funeral Director’s signature.
- This permit must be issued in person; it is not available online or by mail.
Non-Communicable Disease Letter Request
A Non-Communicable Disease Letter is required documentation certifying that a decedent’s remains (burial or cremation) do not pose any health threats from a communicable disease, ensuring to be safe for international transport. This letter must be notarized by the local registrar.
Note: A disposition permit, embalming records, or certificate of cremation will be required for the letter to be issued.
A Non-Communicable Disease letter can only be issued in-person by:
