Have Your Voice Heard in the 2025 Community Health Assessment

MONROE, N.C. (Feb. 21, 2025) – Union County's Human Services Agency is again partnering with Novant Health, Atrium Health and an expanded group of community partners, known as the Central Carolina Community Collaborative, to conduct the 2025 Community Health Assessment (CHA). This year, the assessment is even more significant, involving 10 local public health agencies, six hospital systems and four United Way agencies to gain a comprehensive understanding of the key factors impacting the overall health and safety of residents across the region.

Your responses are anonymous and will be compiled with others from your age group to help determine our communities' physical, mental and environmental health needs. The results will play a critical role in shaping programs and policies to improve health outcomes for Union County residents.

How to Participate

Why the Community Health Assessment Matters

In addition to being a best practice, the State of North Carolina requires the CHA for local health departments to receive funding and Public Health Accreditation. The process examines primary and secondary evidence to develop a factual reflection of population health resulting from emerging trends and epidemiological statistics. The CHA is required to be completed on either a three- or four-year cycle. Union County completed assessments in 2012, 2016, 2019 and 2022. View previous reports on our Community Health Reports page.
